Please fill out the new patient paperwork (and any other paperwork requested) and bring it to your appointment. If you don't have a printer, you can email the forms to health.within.chiropractic@gmail.com.

If your paperwork has already been filled out, please arrive at the time of your appointment.
If you need to cancel or reschedule for any reason, please give a 24 hour notice.
Location
We are located in the Red Wing Armory building in Red Wing, Minnesota.
Enter the building from the front doors facing Plum Street, walk up the first set of steps and straight across the main floor until you see our door.
